General Insurance Information

SCVSDA clubs and activities are insured by Markel Insurance Company through a program coordinated by United Square Dancers of America (USDA). This insurance includes both liability and accidental medical coverage.

Insurance for Dance Sites

Most sites require insurance, and they will require a certificate of insurance showing coverage for a particular club and site before they will give final approval for use. Some sites may require additional endorsement pages, or special wording on the certificate itself.

SCVSDA clubs/committees that need a certificate of insurance should contact Insurance Chair Joe Dehn , 408-774-1570.

Accident Report

Each club should have available a copy of the Club Accident Report form at every regular club night and special event. This document is what is required by our insurance company to start to deal with a claim, and it should be part of the club's regular paperwork that is carried to dances.
